- Form 1 is to be completed when the researcher will collect data directly from human participants.
- Form 2 is for research that uses secondary data only.
- Form 3 is for conceptual or desktop research that uses data that is already in public domain. The review of this research application is very short and ethics approval can be obtained in less than a week.
- Form 4 is for requesting to make amendments to research that was previously approved and/or for requesting an extension of research ethics approval period.
- Form 5 is for class approval, and it is usually completed by the lecturer. Class approval is practiced mainly for honours and undergraduate research projects. Because of the high number of students involved in these projects, it would be practically impossible to review thousands of applications for ethics review. What happens is that the lecturer will decide on four (4) or five (5) different topics and the students will choose one of them, the lecturer will then apply for ethics approval for the 4 or 5 different research topics, for the class.
